If a patient has a missing tooth clause, will the insurance pay for the retainers (abutments) on the bridge and not the missing tooth?

The retainers (abutments) should be covered if they qualify for crown coverage independent of the bridge.  It is common for carriers to deny the entire bridge due to a missing tooth clause but pay for one or more retainers upon appeal when the restorative need is demonstrated.
